CROS & BiCROS Hearing Aids: The Definitive Guide for Single-Sided Deafness in India

Single-sided deafness (SSD) means one ear has no usable hearing while the other is normal or aidable. Causes include Sudden Sensorineural Hearing Loss (see the SSHL emergency guide), acoustic neuroma surgery, Ménière's disease progression, meningitis, congenital atresia and mumps. In adult clinics, sudden SSHL is the leading cause of new SSD cases.

The impact is larger than the audiogram suggests. Without two ears feeding timing and loudness cues to the brainstem, sound localisation collapses (which car is honking?), speech-in-noise separation plummets (the cocktail party problem), and safety awareness on the affected side falls sharply. Adults with SSD report constant micro-adjustments — turning bodies, repositioning at tables, avoiding restaurants — that accumulate as chronic listening fatigue.

CROS (Contralateral Routing of Sound). A transmitter worn on the non-hearing ear picks up sound and wirelessly streams it to a receiver device on the better ear. The better ear has normal (or near-normal) hearing and simply plays the streamed sound — the wearer hears the world's left side through their right ear, restoring 360-degree awareness.

BiCROS (Bilateral CROS). If the better ear also has some hearing loss, the receiver device amplifies both the local sound (for the better ear's own loss) and the streamed sound from the dead ear. Same principle, one system.

CROS is not simply half a hearing aid. Fitting requires: full audiometry on both ears (dead side and better side), speech recognition testing on the better ear to confirm CROS is the right approach (occasionally a cochlear implant on the dead side is a better long-term answer — see cochlear implant vs hearing aid), device pairing and REM verification on the better ear (see REM guide), and structured adaptation coaching (the streamed side sounds oddly 'flat' at first while the brain habituates).

SSHL survivors who progress to permanent unilateral loss are common CROS candidates. Others include long-term Ménière's disease patients whose affected ear has become profoundly deaf, and acoustic neuroma survivors after translabyrinthine surgery.

CROS restores: 360-degree environmental awareness (someone speaking on your dead side is now heard), safer navigation on the dead side, reduced listening fatigue in noise, better restaurant conversation when the noisy speaker is on the dead side.

CROS does not restore: True binaural sound localisation (both ears must feed the brainstem for that; a single-ear receiver cannot recreate directional timing cues). Cochlear implantation on the dead ear can restore true binaural processing, but requires major surgery and 6–12 months of rehabilitation.
