Cochlear Implant vs Hearing Aid: When Amplification Is Not Enough

The clinical distinction — amplification versus stimulation

Hearing aids amplify sound acoustically. Sound entering the ear canal is louder and cleaner, but the wearer's damaged hair cells still have to transduce it into neural signals. When enough hair cells have died — typically severe to profound sensorineural loss with poor speech discrimination scores despite optimal amplification — no amplifier can produce speech clarity because the transduction machinery is gone.

Cochlear implants bypass this problem entirely. A surgically-implanted electrode array in the cochlea delivers electrical stimulation directly to the auditory nerve. An external processor (magnetically coupled to a subcutaneous receiver) converts sound into stimulation patterns. The brain learns to interpret these patterns as speech and environmental sound over 6–12 months of rehabilitation.

Adult candidacy — the standard Indian criteria

Adults are considered for cochlear implantation when they meet all of the following:

  • Bilateral severe-to-profound sensorineural hearing loss (typically ≥70 dB in the better ear).
  • Aided speech recognition scores below approximately 50% (60% in some centres) on sentence-in-quiet tests with best-fitted hearing aids, worn for at least 3 months.
  • Anatomically viable cochlea and auditory nerve confirmed by high-resolution CT temporal bone and MRI.
  • Absence of medical contraindications to surgery.
  • Realistic expectations and commitment to post-implant auditory rehabilitation.

Post-lingual adults (those who lost hearing after developing language) achieve the best outcomes — a well-programmed implant restores telephone-level speech comprehension in most cases within 6–12 months. Pre-lingual adults have poorer speech outcomes but benefit substantially in awareness and lip-reading enhancement.

Paediatric candidacy — implant early, implant well

Paediatric cochlear implantation is one of the highest-impact medical interventions in modern medicine. Children with bilateral severe-to-profound congenital hearing loss implanted before 18 months of age typically develop age-appropriate spoken language. Later implantation compresses this window rapidly — after age 3, the window for near-normal spoken language outcomes narrows steeply, and after age 7 outcomes for a previously non-signing child are limited.

The Indian ADIP paediatric cochlear implant scheme funds surgery, device and rehabilitation at empanelled tertiary centres for eligible children up to age 6. This scheme has funded over 5,000 paediatric implants across India since 2014 and is one of the most successful public health programmes in Indian audiology.

Indian devices and costs

The major cochlear implant brands available in India are Cochlear (Nucleus), Advanced Bionics, MED-EL and Neurelec. Total private cost — device, hospitalisation, surgery and rehabilitation — ranges from ₹6 lakh to ₹15 lakh depending on brand and centre. ADIP-supported paediatric surgery is essentially fully subsidised for eligible families. Adult implants for BPL-certified families are supported at some centres through CSR and hospital charity funds; check AIIMS, PGIMER, CMC Vellore, Amrita Kochi and KEM Mumbai for current schemes.
