Sudden Sensorineural Hearing Loss (SSHL): The 72-Hour Medical Emergency
What Sudden Sensorineural Hearing Loss actually is
Sudden Sensorineural Hearing Loss (SSHL, sometimes called sudden deafness) is defined by American and Indian ENT bodies as a hearing loss of at least 30 decibels across three connected audiometric frequencies that develops over 72 hours or less. It affects roughly 5 to 27 per 100,000 people per year, and its true incidence is under-reported in India because patients often mistake it for wax buildup or dismiss it entirely for the first crucial week.
The mechanism is not fully understood, which is why SSHL is treated as an emergency even when the exact cause cannot be identified. The dominant hypothesis is a viral, vascular or autoimmune insult to the cochlear hair cells or the vestibulocochlear nerve. In about 90% of confirmed cases, no specific cause is ever isolated; this is called idiopathic sudden sensorineural hearing loss (ISSNHL). The critical clinical point is that idiopathic or not, the treatment window is measured in days, not weeks — and outcomes worsen sharply after the two-week mark.
The four red flags that mean SSHL — not wax
1) Unilateral onset. SSHL almost always strikes one ear. Bilateral sudden loss is rare and points to systemic causes (autoimmune, ototoxic drug exposure). If both ears feel blocked overnight after a swim or a cold, wax or middle-ear fluid is far more likely.
2) Speed of onset. Wax and fluid build over weeks. SSHL is often noticed on waking or during a single phone call. Patients frequently describe the exact minute it happened.
3) Associated tinnitus and aural fullness. 90%+ of SSHL cases carry loud tinnitus in the affected ear at onset, and around 40% report vertigo or spinning within the first 48 hours — a marker of poorer prognosis because the vestibular apparatus is involved.
4) The tuning-fork test. Simple bedside Rinne and Weber tuning-fork tests, done in any ENT clinic in five minutes, distinguish conductive from sensorineural loss with ~90% reliability and immediately triage the emergency.
Why 72 hours matters — the treatment window
The primary evidence-based treatment for SSHL is systemic oral corticosteroids — typically prednisolone 1 mg/kg/day (max 60 mg) tapered over 14 days — started within seven days of onset. American Academy of Otolaryngology–Head and Neck Surgery (AAO-HNS) clinical practice guidelines, which Indian ENT departments follow, recommend initiating steroids within the first two weeks, with the strongest response documented when treatment starts inside 72 hours.
Meta-analyses show hearing recovery rates of roughly 60–70% when steroids are started within the first week, falling to 30–40% after two weeks and approaching spontaneous rates (about 30%) after a month. The physiological reason is the finite viability of stressed hair cells: extended cochlear inflammation and metabolic depletion cause irreversible sensorineural cell death.
Intratympanic steroid injection (dexamethasone or methylprednisolone delivered through the eardrum into the middle ear) is offered as first-line therapy for patients with contraindications to oral steroids (uncontrolled diabetes, active peptic ulcer, glaucoma) and as salvage therapy for patients who fail oral treatment. Multiple randomised trials in ENT clinics across Delhi, Mumbai and Chennai show intratympanic steroids match oral outcomes at 3 months with lower systemic side effects.
Diagnostic workup — what a good ENT will order
An adequate SSHL workup performed on day one is essential: pure-tone audiometry (PTA) with air and bone conduction, speech audiometry, tympanometry, and otoacoustic emissions. This audiogram is the baseline that every future recovery decision is measured against.
Once SSHL is confirmed, an MRI of the internal auditory canal (IAC) with contrast is the standard next step to rule out vestibular schwannoma (acoustic neuroma), which presents as sudden or fluctuating unilateral hearing loss in 10–20% of cases. Blood tests for autoimmune markers (ANA, ESR), syphilis serology and a lipid panel are routine. Doppler studies of vertebral circulation may be added if the patient has cardiovascular risk factors.
A poorly-run workup that skips MRI is the single biggest missed opportunity we see referred from smaller centres in North India. If you are told 'take these drops, no need for further tests,' seek a second opinion at a tertiary ENT department the same day.
Recovery, hearing aids and long-term follow-up
About 30% of SSHL patients recover fully with treatment, 50% show partial recovery, and 20% have permanent significant loss. The strongest positive predictors are: age under 60, low-frequency involvement only, absence of vertigo, mild-to-moderate loss at baseline, and treatment initiated in the first 72 hours.
Patients with residual sensorineural loss after 3 months are candidates for hearing aid fitting.
